We are committed to protecting the privacy of children. You should be aware that the Community Care website is not intended or designed to attract children under the age of 14. We do not collect personally identifiable information from any person we actually know is a child under the age of 18 and we do not allow children under the age of 18 to access our secure site. We do allow adults to access the secure site on behalf of their minor children, however we do not share any information on the child that has not first been provided to us by the adult user.

Community Care holds the privacy and security of our members and providers among its highest priority. Our security model strives to assure that you control who can see your information and can feel comfortable storing your private health-related information on our site.
Your secure login process adds a layer of security to our already secure process.
Security Policy
Community Care is committed to complying with all federal, state,
and local laws, as well as applicable regulations,
standards, and guidelines established by government agencies and accepted
accrediting organizations.
To access information in our secure portals, we will provide a
secure transmission method for you to send us personal information,
including (but not limited to) primary personal information
(such as name and contact details). Once you have successfully
registered for one of our secure portals, we will ask you for an
identifier (e.g., Member ID, User ID, website password, etc.) and a
password to authenticate your visit to the site.
While such secure transmission methods provide reasonable
protections against unauthorized access, if you have concerns
regarding the transmission of sensitive information (such as medical
information), you should consider using non-electronic communication
methods.
We have implemented security policies, rules, and technical

Community Care Use Of Data Entered On The Site
When a member enters information via a survey on our web site,
he/she is able to request that a specific provider who has registered
for the site be granted permission to view those data elements.
Anytime a member completes an assessment or survey on the site,
Community Care staff may use that information. We will store the
records in a database that has a direct association between the
member's ID number and the information that is entered on the site.
We will not use any information on the site to decide if a
requested service is medically necessary. We may use the
information to determine if specific services are helpful to
specific members or by putting the information together with
other people's information (aggregation) to determine how effective
a set of services or specific provider may be. Our intent for this
is to be able to see what types of services work best for
our members.
